Directing programs through delegation of responsibility to administrative and supervisory personnel according to their abilities and defined responsibilities

Minimum of five years of supervisory experience in a home health and/or hospice care

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

Full-time employment; 8-hours/day; 40 hours/week.

The Registered Nurse demonstrates proficiency by exhibiting the following skills, competencies, and behaviors:

Ensures plan shows multidisciplinary planning, consultation, and education.

Ensures effective development and completion of discharge plan including discharge barriers and patient/family education.

Upholds Kaiser Permanente's Policies and Procedures, Principles of Responsibilities and applicable state, federal and local laws.

Identifies learning barriers. Provides non-complicated teaching following the registered nurses plan for education.

Evaluates and reports environmental safety concerns to the RN case manager.

Provides therapeutic patient care in accordance with the patients established by the physician and a registered nurse within scope of practice.

Transcribes physicians' orders as directed by policy.

Give specified medication orally, hypodermically, intramuscularly and topically.

Experience with utilization management systems or clinical decision making tools such as Medical Coverage Guidelines (MCG) or InterQual.

Working collaboratively with the audit team to identify and obtain approval for particular vulnerabilities and/or cases subject to potential abuse

Work in partnership with our clients, CMD colleagues, and other contractors on improving medical policies, provider education, and system edits

Strong preference for experience performing utilization review for an insurance company, Tricare, MAC or organizations performing similar functions

Experience with ICD-9, ICD-10, CPT-4 or HCPCS coding.

Knowledge of insurance programs program, particularly the coverage and payment rules.

Location: 9 days REMOTE, 1 day in person

Experience answering phones in a busy environment

Assist guests with navigation of the office

Must have current and active California Home Health Aide Certification.

Five years of experience in home hospice, hospital, or nursing home setting.

6 months Previous patient experience preferred

Certified Home Health Agency certification required

Provide personal care to patients in their homes

High School diploma or GED required

Meets daily average productivity standards set by the agency. Participates in Quality Management activities.

Essential Job Duties And Responsibilities

Offers and assists with bedpans and urinals.

Takes and records oral, rectal, and auxillary temperatures, pulse, respiration, and blood pressure when ordered with appropriate completed/demonstrated skills competency.

Demonstrates knowledge of state, federal, local and accreditation regulations for the delivery of hospice services.

Current and active California Home Health Aide Certification.

Must be willing to work in a Labor Management Partnership environment

Performing household services, which will facilitate the patient's self care at home and are necessary to prevent or postpone institutionalization

Reports stated or observed changes in patient status to case manager

Functions independently with supervision by case manager for each patient every 14 days as needed

Supervisory Responsibilities: This job has no supervisory responsibilities

Minimum six (6) months of Home Health experience

Equivalent experience will be accepted in lieu of the required degree or diploma.

1 year experience performing duties as an aide in a home care setting or other health care related facility.

5 years direct patient care experience.

Working knowledge of medical terminology and health care practices is required as well as understanding of universal precautions.
