Rn Home Care Visit Nurse Jobs in Alabama

Requires use of electronic mail, time and attendance software, learning management software and intranet.

Maintains exemplary attendance and availability according to department need and rotates management duty.

Provides skilled nursing care to an assigned caseload of patients.

Performs skilled assessments and documents findings in the OASIS accurately and timely.

Communicates regularly with the Home Health RN Care Manager regarding patients, progress toward goals, barriers to care and outcomes.

Assess patient’s outcomes and offer suggestions on ways to improve outcome scores.

Interacts with patient/family and caregivers in a supportive, mature, knowledgeable manner and provides compassionate, safe and organized care.

Performs compliance requirements as outlined in the Employee Handbook

Follows physician orders and 485 Plan of care.

Performs wound care measurements weekly.

Communicates regularly to the RN regarding patient problems & progress toward goals.

Five years of professional nursing experience including three years of administering, managing, or supervising a home care program or

Six years of professional nursing experience including three years of administering, managing, or supervising a home care program or

An evaluation of Training and Experience as shown on the application.

The term “regionally accredited school” refers to an institution of higher education accredited by one of the six

Bachelor’s degree from an accredited* college or university in Nursing.

Overnight travel is required for some positions.
