The Home Health Visit Nurse works collaboratively with the Home Health RN Care Manager and other disciplines, providing skilled care, education, treatment, and emotional support to patients. Monitors and assesses the care provided while insuring the patient attains and maintains their highest level of functioning. Insures that the Physician and all members of the care team are aware of pertinent changes in the patient’s condition and progress toward goals. Collaborates with the Home Health RN Care Manager insuring that the established plan of care is appropriate and is implemented and/or modified based on the patient’s responses to treatment.
- Assess patient’s outcomes and offer suggestions on ways to improve outcome scores.
- Educates patients and their families on proper home health care strategies and procedures.
- Measures wounds weekly.
- Provides skilled nursing care to an assigned caseload of patients.
- Reviews the plan of care of assigned patients to insure that the physician orders are being followed and that the patient receives the care required to achieve established goals.
- Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation.
- Performs compliance requirements as outlined in the Employee Handbook
- Participates in the performance improvement plan and process to ensure positive patient outcomes
- Must adhere to all DCH Health System policies and procedures.
- Evaluates, and revises the plan of care in collaboration with the Home Health RN Care Manager as appropriate, based on patients identified needs and progress toward goals.
- Communicates regularly with the Home Health RN Care Manager regarding patients, progress toward goals, barriers to care and outcomes.
- Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self.
- Maintains an accurate and current list of each patient’s medications.
- Performs skilled assessments and documents findings in the OASIS accurately and timely.
- Establishes positive working relationship with physicians and physician’s office staff and keeps them informed of patient’s problems and progress towards goals as appropriate.
- Actively participates in IDT meetings.
- Instructs and supervises the patient and family in the care of the patient and maintenance of a healthy environment for the patient.
- Requires use of electronic mail, time and attendance software, learning management software and intranet.
- Participates in on-call rotation
- All other duties as assigned.
- Monitors the care, treatment, and service provided by other disciplines (SN, PT, OT, ST, MSS, RD, HH Aide) to insure patient receives the care required to improve their health status.
- Encourages a safe environment by promoting adherence to established protocols, policies and procedures.
- Regularly prepares and presents staffing and outcomes metrics with action plans.
- Executes progressive discipline up to and including termination
- Maintains effective interdepartmental relationships by working with other unit leaders and departments to define and work toward common goals
- Responsible for assessing and meeting own professional education needs.
- Manages front line operations of area of accountability related to meeting/exceeding departmental goals, 5 keys of excellence, DCH Mission and Vision.
- Serves as clinical resource for staff, especially for new employees and during complex patient care situations.
- Maintains exemplary attendance and availability according to department need and rotates management duty.
- Observes and practices DCH policy concerning patient confidentiality including on-line information and the Medical Record on file.
- Promotes efficient patient flow including timely discharges and admissions.
- Adheres to productivity, revenue and expense goals.
- Effectively implements current techniques and/or programs to improve patient care processes.
- Communicates information verbally and in writing to appropriate persons.
- Assures compliance with various quality standards and regulatory agency requirements (The Joint Commission, CMS, Standards of Nursing Practice, PI process, value-based purchasing, Structural Indicators etc.)
- Performs initial and annual competency per job class.
- Provides direct patient care when needed.
- Demonstrates knowledge of documentation and down-time procedures.
- Assesses, plans, implements, and evaluates programs for in-service and orientation.
- Maintains records related to orientation in-service and unit competency assessments.
- Regularly rounds on patients, physicians, and employees with focus on clinical safety, quality compliance, patient experience, satisfaction, physician and employee satisfaction and staff performance.
- Interviews, selects, hires, and retains employees
- Assures compliance with policies and procedures
- Ensures care is delivered appropriately by orienting staff in accordance with Standards of Practice, review of clinical record for appropriate documentation, and review of documentation of appropriate plan of care. Leads staff huddles to communicate important, “just in time” information to all staff on duty.

- Coordinates unit activities and provides leadership for unit staffing and patient assignments and responds to and provides leadership during crisis and emergency situations.
