Performs focused interview to identify specific patient needs.

Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.

Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Other Licensed Practical Nurse (LPN) duties as assigned.

Meets all training requirements in accordance with state and federal laws when applicable

Possess and maintains current CPR certification in accordance with state regulation

Current Home Health Aide certification required at all times.

Attending interdisciplinary team conferences when care plan is reviewed.

Pay: $14.00 - $16.00 per hour

Minimum of one year experience as a Home Health Aide required

Must possess a valid NY State Home Health Aide Certification

Knowledge of primary care medicine requirements (state specific).

Basic computer skills and/or clerical experience.

Less than 2 years of technical experience

Knowledge of procedures and techniques involved in administering simple treatments and providing related patient care

EMR (Electronic Medical Record) experience

High School Diploma or equivalent

Assist and educate client and family on activities of daily living, including bathing, grooming, toileting and elimination, and adequate nutritional intake.

Promote a safe environment and maintain unobstructed pathways while providing care.

Assist with ambulation, transfers, and/or range of motion exercises.

Recognize, document, and report changes in client condition and safety to supervisor.

Complete other assignments as requested and assigned.

Data entry into HHA Exchange

HS Diploma/GED Associates preferred; experience accepted as substitute

2-5 years of experience in the home care industry; or related industry.

Knowledge of home care concepts, practices and procedures for home care programs

Salary Range : $16 - $19 / hour commensurate with experience

A minimum of 10 years of senior management experience is required.

HIGHLY Preferred: Candidates with extensive homecare (non-skilled / ADL) & IDD experience.

We will consider exceptional talent with Skilled Home Health experience as well.

Successful candidates must also possess a strong knowledge of business strategy and finance.
