1 Year One year of acute/psychiatric hospital experience, discharge planning/case management experience.

Master's Degree In Social Work from an accredited graduate program.

AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED

California BBS Associate Clinical Social Worker (ASW) - CA Board of Behavioral Sciences -REQUIRED

Hourly Pay Range (Minimum - Midpoint - Maximum):

Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.

Explains hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents as requested.

At least one (1) year of social work experience in a healthcare setting highly preferred.

BSW from an accredited school of social work is minimum education required but must be supervised by MSW.

Participates as a member of the IDT, including development and implementation of the plan of care.

Assists in identifying the need for intervention of other IDT members.

Education, Knowledge, Skills and Abilities Required:

Perform initial psychosocial and bereavement assessment of patients that assists the interdisciplinary teams plan of care and treatment goals.

Assure plan is received and updated as needed. Assure that risk factors and treatment goals are documented.

Instruct and counsel patients/families in treating and coping with social and emotional responses related to recovery and/or terminal needs.

Assure documentation of assessments, interventions and planning meet organization timelines.

Provide follow-up to patient/family to ensure implementation.

A true desire to satisfy the needs of others in a fast paced environment.

Must have physical stamina to lift moderate amounts of weight.

Ability to work a flexible schedule including evenings, weekends and holidays

The budgeted rate for this role is $14.27.
