APPROXIMATE ANNUAL SALARY - $45,184.32 to $66,043.44 PAY GRADE: 29

Work Type: A Permanent, full time vacancy

Bachelor’s Degree in social work with experience in case management and family engagement/supports/advocacy

Maintaining accurate and detailed documentation of case management services within ChildPlus software and other required data management systems.

Recruit eligible children, including those with disabilities, and assist families in completing registrations.

Monitor attendance and discuss with family the importance/benefits of regular attendance.

Make referrals to the Disabilities/Mental Health Specialist on children with disabilities, prior to enrollment.

Obtain complete health history and parental consent on all health/education screenings.

Monitors family engagement and serves as the primary contact for enrolled children and their families. (Children 18 months – 4 years)

Acts as point of contact for children and families referred by TEIS during screening and intake

Assist the interdisciplinary clinical team with service availability and match for child and family needs

Develops and participates in collaboration with group therapy services for children 18 months-4 years

Greets staff and clients and conducts health screenings and other duties with professionalism.

Performs other duties as assigned
