Enhanced Care Management Care Navigator Jobs in Iowa

Three (3) years of experience in case management preferred

Drive patient and families in their own care and to support self-management

Experience in a dialysis center or transplant center preferred

Experience with Medicare and Medicaid preferred

Handle in and outbound calls delivering world-class service to our members

Engage members into HealthMap’s Kidney Health Program

Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.

Assist with coordination of activities related to insurance pre-certification/authorization.

Provide counseling to patient, participant or their representative regarding pre-service requirements and instructions.

Gather necessary demographic, insurance and clinical information from patient and enters into appropriate database.

Seek appropriate resources to resolve issues about the type, date or location of prescribed procedures.

Schedule patient procedures in a manner that most efficiently utilizes the patient's time and clinical resources.

Strong written and oral communication skills including ability to be able to communicate effectively with all level of management and employees

Strong time-management skills and keen eye to detail

Establish positive partnerships across business groups both inside and outside of the organization

Operate various business instruments, including computers, printers/scanners/fax machines, phones requiring manual dexterity

Sit and concentrate for long periods of time to compute, examine, and interpret data

Read paperwork and computer monitors, requiring visual acuity

Degree in Medical Assistant or Medical Office experience preferred

Foundational knowledge base in medical terminology

Ability to multitask, with prioritization skill set

Ability to acknowledge and handle multiple personality and character traits in a polite and respective manner

Complete daily call assignments by utilizing call scripts to communicate to patients and providers regarding their recommended upcoming and/or overdue follow-up.

Answer inbound calls from patients and providers and address any questions and/or concerns.
