• Ensuring that all necessary paperwork is completed prior to discharge.
• Ensuring that the patient is medically stable prior to discharge.
• Working with the patient's family and other healthcare providers to ensure a smooth transition from the hospital to the patient's home.
• Coordinating the discharge process with other departments, such as social work, pharmacy, and laboratory.
• Assisting with the discharge planning process, including arranging for follow-up care and providing patient education.
• Ensuring that the patient is discharged in a timely manner.
