Excellent leadership and team management skills

Minimum of five (5) years of previous experience in social services and discharge planning within an acute care hospital setting

Strong technical knowledge of social work and discharge planning processes

Bachelor's Degree in a relevant field

Valid license and current registration issued by the New York State Board of Nursing

Basic computer skills. Knowledge of Microsoft Office 97: Microsoft Word, Access and Excel, preferred.

Performs related duties as required.

Bachelor's degree in Nursing, required. Master's Degree, preferred.

Current license to practice as a Registered Professional Nurse in New York State.
