Community Living Coordinator Jobs

Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.

Excellent project management skills, with a track record of developing and implementing new or improved systems and processes.

Producing educational or other relevant materials for key stakeholders coming from the CTG initiative; and

A four-year degree and/or equivalent of 8-10 years’ work experience in a related role or field; master’s degree preferred.

Experience providing technical assistance, developing and disseminating resources, and managing cohorts or communities of practice.

Experience in addressing racial wealth inequality, especially for BIPOC communities.

Strong supervision experience demonstrating an inclusive, collaborative leadership style.

Provide independent living skills training

Conduct public education and awareness activities

Monitor and adhere to established program budgets, including grant requirements attributed to the program.

Conduct initial assessment to determine consumer’s individual needs

Assist consumer in developing initial, intermediate and long range goals

Track and monitor consumer progress
