Knowledge of and ability to navigate community resources

Ability to manage multiple priorities in a fast-paced environment

Experience working with members/patients with medical and mental health co-morbidities

Must have Community Health Worker certification or complete CHW training and achieve certification within first six (6) months of hire.

Is a resident of Tippecanoe County and/or has extensive knowledge of community members, the neighborhood served, and available community resources.
