Overview The RN Case Manager Weekends Home Health is responsible for providing comprehensive care to patients in their homes. This includes assessing the patient’s condition, developing a plan of care, providing direct care, and coordinating with other healthcare professionals to ensure the patient’s needs are met. The RN Case Manager Weekends Home Health also provides education and support to the patient and their family. Detailed Job Description The RN Case Manager Weekends Home Health is responsible for providing comprehensive care to patients in their homes. This includes assessing the patient’s condition, developing a plan of care, providing direct care, and coordinating with other healthcare professionals to ensure the patient’s needs are met. The RN Case Manager Weekends Home Health also provides education and support to the patient and their family. The RN Case Manager Weekends Home Health is responsible for ensuring that the patient’s care is provided in a safe and effective manner. The RN Case Manager Weekends Home Health is also responsible for documenting the patient’s care and progress. Job Skills Required
