Case Management Discharge Coordinator Jobs in Ohio

Excellent interpersonal and communication skills (verbal and written) with providers, ship personnel, employees, internal departments, and management

Address simple queries or concerns raised during the Case Management Process or the Seafarer Health process

2+ years of work experience with similar scope and responsibilities is required

College degree may substitute for work experience

Ability to apply analytical and logistic skills; maintain attention to detail and accuracy

Proficiency in the use of computer business applications with working knowledge of Word and Excel

Case management and discharge planning experience.

Be clinically and culturally competent/responsive with training and experience necessary to manage complex cases in the community across child-serving systems.

2+ years of experience in behavioral health, social services, or human services

2+ years of experience with personal computers, keyboard and multi-system navigation, and MS Office Suite applications (Outlook, Word, Excel, SharePoint).

2+ year of experience with Ohio delivery systems, including local community networks and resources.

Enhancement of Medical Appropriateness and Quality of Care:

Experience with a mental health/behavioral health population or prior case management required

Bachelors Degree (preferred) in a social services field (Required)

Acute Care Unit-Discharge Planner (Social Services)

Provide discharge planning and support to the social worker/therapist on the in-patient unit.
