They work with clients to identify and address social, emotional, and economic needs, and provide resources and support to help them achieve their goals. Detailed Job Description Care Management Social Workers are responsible for providing social work services to individuals, families, and communities in need of assistance. They assess the needs of their clients, develop individualized plans to address those needs, and provide resources and support to help them achieve their goals. They also provide counseling, crisis intervention, and advocacy services. They may also provide referrals to other social services, such as housing, employment, and health care. Job Skills Required

Job Qualifications
• Bachelor’s degree in social work or a related field
• Master’s degree in social work or a related field preferred
• Licensure as a social worker in the state of practice
