They work with individuals, families, and other professionals to develop and implement treatment plans that are tailored to the individual's needs. They also provide education and resources to help individuals and families manage their mental health and/or substance abuse issues. Detailed Job Description Care Coordinator Behavioral Health is responsible for providing comprehensive care coordination services to individuals with mental health and/or substance abuse issues. This includes assessing the individual's needs, developing and implementing treatment plans, providing education and resources, and monitoring progress. They also provide crisis intervention services, coordinate referrals to other professionals, and provide support and advocacy for the individual and their family. Job Skills Required
