Job Description
Responsibilities
Collaborate with healthcare professionals to ensure the compliance of medication prescribed by physicians or advanced care providers.
Facilitate partnerships among patients, their personal providers, and their families when appropriate to enhance the quality of care.
Provide psychotherapy services and diligently track progress between provider appointments.
Offer valuable information to patients about available community services and resources to support their overall well-being.
Review and maintain care plans and charts, coordinating closely with the care provider to ensure adherence to established plans.
Requirements
Demonstrated experience as a Behavioral Health Specialist/Clinical Social Worker in a clinic setting.
Ability to work effectively within a multi-disciplinary team to deliver comprehensive patient care.
Profound knowledge of health and patient care regulations, ensuring strict compliance.
Exceptional communication skills, both verbal and written.
Unwavering commitment to ethical standards in all aspects of professional practice.
Strong team spirit and a consistently positive attitude toward colleagues and patients.
