This position is responsible for providing comprehensive case management services to clients, including assessment, planning, coordination, monitoring, and evaluation of services. The Care Connection Specialist II will also provide advocacy and education to clients and their families. The Care Connection Specialist II will:

• Develop and maintain relationships with clients, families, and other stakeholders
• Assess client needs and develop individualized care plans
• Coordinate and monitor services provided to clients
• Advocate for clients and their families
• Educate clients and their families about health care services and resources
• Provide support and guidance to clients and their families
• Monitor client progress and evaluate outcomes
• Maintain accurate and up-to-date records
• Participate in team meetings and other professional development activities

Job Qualifications

• Knowledge of relevant laws and regulations
• Ability to work with diverse populations
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
• Proficiency in Microsoft Office Suite
