Cardiovascular Clinical Nurse Navigator Jobs in Ohio

Demonstrated experience with program development/management, including data collection and quality monitoring activities.

Continuously evaluates and monitors all aspects of clinical management and communication to recommend modifications in approach.

Functions as a change agent by helping to successfully implement management decisions and other hospital and departmental changes and objectives.

Experience providing in-service education for hospital staff as well as presentations for consumers/public.

Performs educational needs assessments and develops, implements, and evaluates educational programming to meet the learning needs of the patients and/or employees.

Supports evidence-based practice by applying knowledge/research findings as reported in the professional literature to clinical practice.

Maintains professional knowledge by affiliating with professional and technical organizations, and participating in applicable continuing education programs, conferences, seminars, and workshops.

Minimum 2 years of relevant work experience

Skill in collecting, organizing and analyzing data.

Triages new patient appointments to the appropriate provider(s) and assures timely scheduling of initial appointments.

Reviews outside medical records for appropriate scheduling.

Assess barriers to care and refers to support services, local, and national organizations when needed.

Graduate of an accredited or state board of nursing approved Registered/Professional Nursing program if less than 3 years' experience.

2 years of nursing experience.
