Bereavement Coordinator Jobs

Education and experience related to the care of bereaved individuals

Provides a consistent high quality hospice experience for every patient and family served by soliciting and identifying patient and family goals

Provides assessment, planning, intervention and ongoing evaluation of psychosocial function to meet assigned patient and family needs

Communicates timely and effectively with the appropriate Team Leader/designee, physicians, patients, families, contracted vendors and community resources

In collaboration with Chaplains, provides bereavement support to families

Sensitivity to the particular needs of those in grief

Master's Degree public health-related or similar field, such as health promotion, health communication, health education, social work, or public healt

Previous experience supporting families with pregnancy loss or the death of an infant.

Previous experience conducting quantitative and qualitative research.

Prior experience working in a government Public Health Agency.

Applicant must have 2 years of relevant experience with the following:

Previous work or research in bereavement support

Experience in budget management, program planning, program development, and strategic planning.

Must be able to work in a Labor/Management Partnership environment.

Minimum three (3) years of counseling experience.

Current professional licensure (LCSW or MFCC), or certification.

Consults and provides resource to designated interdisciplinary team members to maximize patient care outcomes and resource utilization.

Ensures compliance with federal, state, and local regulations.

Evidence of experience and/or education in dealing with issues of grief and loss.

Evidence of experience or education in counseling individuals dealing with issues of grief and loss.

Provides education about bereavement services to members of the IDT.

Provides bereavement counseling and support, based on personal qualifications, when needed.

Bachelor’s degree from a school of social work accredited by the Council on Social Work Education, or

Bachelor’s degree in an applicable field such as theology of education, psychology, or counseling; AND
