Job Description
Responsible for the home health, hospice, and bereavement social services programs. Provides fiscal management, designs and provides training, and monitors the on-going volunteer activities.

Essential Functions:
  • Consults and provides resource to designated interdisciplinary team members to maximize patient care outcomes and resource utilization.
  • Responsible for developing and maintaining community relations and all community liaison activity.
  • Designs, implements, monitors and evaluates the bereavement program services on an on-going basis for effectiveness and applicability. Oversees the bereavement volunteers.
  • Monitors and maintains the budget. Identifies and recommends opportunities for cost savings.
  • Participates in the departmental quality assurance program, evaluating the social services and bereavement activities.
  • Provides primary lead for outside presentations, convening informational workshops, meetings and training sessions.
  • Kaiser Permanente conducts compensation reviews of positions on a routine basis. At any time, Kaiser Permanente reserves the right to reevaluate and change job descriptions, or to change such positions from salaried to hourly pay status. Such changes are generally implemented only after notice is given to affected employees.
  • Ensures compliance with federal, state, and local regulations.
  • Provides supervisory oversight regarding skills development, individual competency, capability and positive outcome participation as a team member in the development of integrated care plans, focusing on collaboration, and results oriented implementation functions.
  • Manages staff to meet administrative expectations in the areas of standards of practice, policies and procedures, annual departmental goals and objectives, quality improvement, and licensure and accreditation.

Basic Qualifications:
Experience
  • Minimum three (3) years of experience in program management, supervision, or oversight responsibilities, as well as direct counseling work with individuals/groups, in a health care setting.
  • Minimum three (3) years of counseling experience.
Education
  • Master's degree in counseling, social work or related field.
License, Certification, Registration
  • Current professional licensure (LCSW or MFCC), or certification.

Additional Requirements:
  • Must be able to work in a Labor/Management Partnership environment.
  • Understanding of new and innovative organizational change processes.
  • Demonstrated expertise in working with grief /loss issues and crisis intervention.

Preferred Qualifications:
  • Experience in budget management, program planning, program development, and strategic planning.
