Detailed Job Description The Behavioral Health Home Health Coordinator will be responsible for providing comprehensive care coordination services to individuals with behavioral health needs. The Coordinator will assess the individual’s needs, develop a care plan, coordinate services, and monitor progress. The Coordinator will also provide case management services, including linking individuals to appropriate services and supports, advocating for individuals, and providing education and support to individuals and families.
