Job Description
The Guidance Center (TGC) is currently seeking Behavioral Health Coordinator to be part of our innovative interdisciplinary team. TGC is a Community Mental Health Center serving Leavenworth, Atchison, and Jefferson counties in Kansas.   • Increase hourly rate of $21.63 to $24.04 per hour; based on previous relevant experience effective September 6th, 2023

Responsibilities For Behavioral Health Coordinator
  • Under the supervision of a QMHP, this position will facilitate the treatment plan process by inviting all involved participants, to include client to develop or update a plan which addresses identifies needs, defines goals and objectives, and action steps which support community living and participation.
  • Enter treatment plan details into electronic record and other documents in a timely manner.
  • Provide face-to-face, phone and written communication with caregivers, other service agencies, housing resources, and medical services to assist with meeting client/family needs.
  • Provide assertive outreach directed toward engaging and maintaining client relationships as well as relationships with caregivers
  • Monitor treatment plans to ensure the plan is current, ensure that updates are accomplished before the plan expires and enlist assistance of supervisor if problems are encountered.
Behavioral Health Coordinator Qualifications
  • Bachelor’s degree required or be equivalently qualified by work experience or a combination of education and work experience in the human services field with one year of experience substituting for one year of education.
  • Covid 19 vaccine or approved medical/religious exemption is required.
