Case management and discharge planning experience

Utilizes case management and quality management processes in compliance with regulatory and accreditation

Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promotes lifestyle/behavior changes to achieve optimum level of health

Knowledge of laws and regulations governing delivery of Medicare services

Through the use of clinical tools and information/data review, conducts comprehensive assessments of referred

Identifies and escalates quality of care issues through established channels

Assist employee-client in making informed choices via patient advocacy in alignment with the Medical Disability Advisor and laws regulating disability management.

Act as a liaison between all parties required in case management to facilitate continuous communication and consensus.

Compassionate with a keen ability to actively listen and build client rapport through exceptional communication and relationship-building skills.

2+ years as a Clinical Case Manager.

Must show evidence of appropriate degree and behavioral health qualifications.

Manage medical disability and accommodation cases, as needed, if within the scope of licensure and clinical expertise.
