Work collaboratively with justice entities, health care providers, and community providers to integrate care and address social determinants of health.

Secure outpatient appointments for individuals and facilitate home and community-based services as necessary.

Facilitate communication between the member or designated representative and member’s healthcare providers

High School Diploma, GED or equivalent, required.

Experience in a school or after-school environment preferred

Strong interpersonal skills and ability to use sound judgement

Establish and maintain a safe and nurturing learning environment

Develop interactive activities to engage students throughout the afternoon (2:30 p.m. to 6:00 p.m.)

Supporting students with supervision and homework assistance.

Accepts responsibility for maintenance and necessary housekeeping duties of the classrooms, common use areas and playgrounds

Employer paid trainings/scholarships to pursue a degree in Education

Minimum of two years documented experience working with an early childhood population.

Passion and skill for working with children.

Special skills in art, music, drama, science/STEM, sports highly desired

Strong oral and written communication skills and basic computer skills.

Willingness to obtain Infant/child CPR and First Aid Certification if not certified
