Overview Adult Case Managers are responsible for providing support and assistance to adults with mental health and/or substance abuse issues. They work with clients to develop treatment plans, coordinate services, and provide resources to help them achieve their goals. They also provide advocacy and support to ensure that clients receive the best possible care. Detailed Job Description Adult Case Managers are responsible for providing support and assistance to adults with mental health and/or substance abuse issues. They work with clients to develop treatment plans, coordinate services, and provide resources to help them achieve their goals. They also provide advocacy and support to ensure that clients receive the best possible care. They may also provide crisis intervention, referrals to community resources, and follow-up services. Job Skills Required• Knowledge of mental health and substance abuse issues
• Ability to assess client needs and develop appropriate treatment plans
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Ability to work with diverse populations
• Knowledge of community resources
• Knowledge of relevant laws and regulations
• Ability to maintain client confidentiality
• Excellent organizational and time management skills
Job Qualifications
• Bachelor’s degree in social work, psychology, or a related field
• Master’s degree in social work, psychology, or a related field preferred
• Licensure or certification in social work, psychology, or a related field preferred
• Experience working with adults with mental health and/or substance abuse issues preferred
Job Knowledge
• Knowledge of mental health and substance abuse issues
• Knowledge of community resources
• Knowledge of relevant laws and regulations
• Knowledge of crisis intervention techniques
Job Experience
• Experience working with adults with mental health and/or substance abuse issues preferred
• Experience in case management preferred
Job Responsibilities
• Develop and implement treatment plans for clients
• Coordinate services and resources for clients
• Provide advocacy and support to ensure clients receive the best possible care
• Provide crisis intervention and referrals to community resources
• Monitor client progress and follow-up services
• Maintain client confidentiality
