Description
At Cocoon House we empower young people, families, and communities in Snohomish County to break the cycle of homelessness through outreach, housing, and prevention. Currently, our Youth Housing program is looking for people to fill the important role of the Young Adult Case Manager (Navigator) who work directly with youth ages from 18-24 residing in Snohomish County who are at risk or experiencing homelessness to appropriate services and housing programs in the county. Coordinated Entry Navigator is the internal name for this role.
Our Young Adult Case Manager....
  • Operate on-site and in Snohomish County to meet the needs of clients.
  • Will Receive referrals from multiple sources and coordinate with navigator staff, ensuring a response in 48 hours.
  • Collaborate and partner with other Cocoon House Programs, law enforcement, resource centers, schools, churches, and detention centers to identify young adults who need applicable services.
  • Support young adults in navigating rental assistance programs, rapid resolution, and other creative solutions.
  • Meet with youth and young adults experiencing homelessness throughout Snohomish County to enroll in Coordinated Entry.
What does a shift look like?
Full-time
  • Case Manage all young adults in order to connect them to resources for emergent needs, community resources, and housing.
  • Attend internal and community meetings and trainings as required.
  • Maintain a cooperative and effective environment free of discrimination, retaliation, and harassment.
  • Collecting and entering data to meet all timelines and deadline. Must maintain organized and complete files.

Pay Scale: $20-22/hrRequirements:
General
  • Acceptable driver's abstract with no more than 2 violations within last 3 years
  • 1 year minimum of experience working with youth
  • Possess a valid driver's license, insurance, and reliable transportation
  • DSHS background clearance
  • Must be 21 years of age or older
  • Proficient in crisis intervention/de-escalation in order to provide proper care to youth and young adults in crisis.
Highly Preferred
  • Spanish-speaking/Bi-lingual
  • Associate's Degree or Higher in social science or equivalent field.
Physical
  • Ability to read and comprehend a variety of materials
  • Be able to hear voices and alarms.
  • Ability to operate phone, computer, and record information for files and logs
  • Ability to effectively communicate with all internal and external clients
  • Ability to travel for various distances through urban or rural environments in various weather conditions
  • Must be able to lift 20-40 lbs.
  • Must have ability to run up and down indoor and outdoor stairwells in case of youth emergency
