• High School Diploma or GED and experience in healthcare services.
• Preference will be given to currently licensed in the State of Illinois as a LPN or Graduate of Accredited Practical Nursing Program.
• Interest and/or background in promoting holistic care.

• Provides accountability, support, motivation and coaching to InCK parents and child on improving health/wellness.
• Assists primary care doctors and school nursed in review, implementation and development of client care.
• Individuals must demonstrate great cultural and socioeconomic sensitivity to populations served
• Conducts Health Risk Assessments, Care Mapping and Care Plans.
• Provides care coordination and holistic wellness services in home, school and community to referred InCK eligible children due to medical necessity including those with medically complex needs or children on anti-psychotics or other behavioral health me
• May do screenings for physical, behavioral or social determinant needs.
