Under general supervision, provides case management services to enrolled population, represents the program in the hospital and the community, advocates and offers supportive services across many systems to individuals who have been victims of violence/ violent crime. Additionally, the case manager provides direct services to a caseload of clients, strengthens the program model by participating in community events, task forces, research components of the intervention, youth violence prevention, and other supportive aspects of service delivery.


Qualifications


Education and Experience


  • LMSW preferred
  • Background in substance abuse, traumatic stress, motivational interviewing, and/ or violence prevention/ intervention preferred.
  • Bachelor’s Degree in sociology, psychology, social work or related field required.
  • Two years in community outreach, health case, social services, or direct client services
