Triage Rn (On Call)

Responsibilities

• Provide an environment which fosters continuous quality assessment performance improvement (QAPI); maintain high standards of patient care, integrity and cost effectiveness.
• Promote effective performance and delivery of after-hours home health; guide clinicians appropriately.
• Triage calls appropriately; refer to local on-call schedule.
• Assist in the orientation of new triage nurses.
• Receive, review and enter patient referral information appropriately. Work with local branch to schedule patient admission during the weekend or holiday ensuring a same day admission philosophy.
• Enter on-call coordination note in patient electronic record for all triaged calls.
• Provide a focused effort to reduce unnecessary hospitalizations and emergency department (ED) trips during after-hours and holidays.
• Contact local leadership appropriately for follow-up on time sensitive issues.

Qualifications

• Must be able to work Friday 5:00pm - Monday 8:00am and on company holidays.
• Must have 5 years of full-time experience as a RN.
• Must have telehealth experience
• Must have 1 year of full-time recent experience in a supervisory home health RN position.
• Must have 2 years of full-time experience in home health nursing.
• Homecare Homebase software/workflow background preferred
• Must be an RN
