Job Summary
The Care Transition Coordinator is responsible to determine estimated Length of Stay (LOS) at admission, using evidence-based workflows and state/national resources, to guide practitioners in developing the overall treatment planning and coordination of patient transition from the acute care setting to home or other subacute care entities. Staff will liaise with patients, families and caregivers to communicate effective patient-centered transition strategies as determined by the Attending and other assigned providers.
Requirements
AHA BLS for Healthcare Providers within 30 days.
Bachelor's degree in a related field and 4 years experience; or equivalent combination of experience and education.
Bachelor's in Nursing or other clinical discipline and/or management preferred.
Clinical licensure/certification, experience and/or training.
Excellent verbal and written communication with outgoing and collaborative personality.
