Job Description
Suburban Home Health is a joint venture between several Suburban Health Organization hospital systems in Indiana. This allows us to work as an extension of our partner hospital systems to service homebound patients in our local communities.

The Triage Nurse responds to all patient requests telephonically during on-call hours (4:30 pm-8:00 am) by providing professional nursing assessment to identify the urgency of patient needs.

  • Communicates and acts as a liaison with patients, family and other health professionals to ensure the needs of the patient are adequately addressed
  • Responds to patient calls within 15 minutes
  • Calls for crisis intervention if appropriate
  • Provides education to the patient and/or family in accordance with the Plan of Care
  • Contacts the regional on-call nurse and provides report when call-back visits are warranted
  • Adheres to agency documentation requirements to provide timely, detailed, accurate documentation of care or services provided
  • Contacts physicians if PRN orders are needed in order for the on-call nurse to perform a call-back visit
  • Accurately documents all on-call activities via the on-call communication log
  • Interviews the patient and/or family to identify triage priority based on available information
  • Consults the Administrator on-call when necessary
  • Minimum of 3 years nursing experience in an acute care setting, preferably home health.
  • Excellent verbal and written communication skills and strong interpersonal skills.
  • Minimum of 1 year home health experience
  • Must possess excellent interpersonal skills to foster a team environment.
  • Ability to prioritize work using fundamental time management skills.
  • Must possess current license from the Indiana State Board of Nursing.
  • Must possess attention to detail skills.
  • Graduate of an accredited school of professional nursing.
