Job Description
  • JOB SUMMARY: The Survey RN is responsible for the performance of timely, accurate mock surveys, as assigned, for Skilled Home Health, Behavioral Health and Hospice providers. This position will assist in the review and approval of plans of correction developed by the Administrator to address survey deficiencies identified during the mock survey. The RN works collaboratively with Operations, Human Resources, Compliance, and other service areas to achieve optimal, accurate survey results.
  • JOB REQUIREMENTS: • EDUCATION: Registered Nurse, BSN preferred. • EXPERIENCE: Minimum three (3) years’ experience in home health and hospice with extensive knowledge of the Medicare Conditions of Participation (CoPs) and other applicable regulations. Experience as a surveyor or leading surveys preferred. • CERTIFICATION/LICENSURE: Certified Registered Nurse. • SKILLS: o Excellent verbal and written communications skills with proven history of ability to collaborate with staff o Demonstrated ability to correctly interpret and apply the CoPs and other applicable regulations for home health and hospice. o Proven history of ability to organize large projects o Advanced knowledge of MS Office applications (Word, Excel) o Demonstrates flexibility during the survey process • TRANSPORTATION: Reliable Transportation. • TRAVEL: Not applicable. • HEALTH STATUS: Meets all applicable agency policies and procedures related to health screening and required testing. KEY ACCOUNTABILITIES: • Ensures that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Survey Preparedness team. • Promotes Elara Caring’s philosophy, mission statement and administrative policies to ensure quality of care. • Maintains patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule. • Performs surveys as assigned for home health and hospice in all states that Elara serves patients in to assess the quality of patient care delivery and determine compliance with the Center for Medicare and Medicaid Services (CMS) CoPs/Accrediting body standards, other applicable regulations. • Supports agency Administrators and leaders when in an actual CMS/Accrediting body survey. • Arrives at mock survey site organized and prepared to initiate mock survey. • Applies professional knowledge, experience, and ongoing knowledge of health care standards while conduction mock surveys. • Conducts daily verbal exit conference with the Administrator summarizing findings and encourages the Administrator to present any information that may change the determination of the potential deficiency. JOB DESCRIPTION TITLE: Survey Registered Nurse DEPARTMENT: Survey Preparedness REPORTS TO: Survey Preparedness Director Survey RN JP728 03/2021 Page 2 of 3 • Conducts a comprehensive exit summary upon the completion of the mock survey giving a verbal review of potential violations of the CoPs/, other applicable regulations and quality of patient care concerns noted during the mock survey. • Applies systems analysis skills and inductive reasoning skills to determine the providers degree of compliance with the CoPs and other applicable regulations and functionality of care delivery systems. • Prepares a written statement of deficiencies documenting any quality of patient care concerns, violations of the CoPs and other applicable regulations, citing the specific regulation found to be deficient and submits to the Administrator within 10 working days following the survey exit review. • Reviews plan of correction submitted by the Administrator within 10 days of receipt and responds with approval or a request for additional actions to correct the deficiency. • Collaborates with operational and clinical leadership regarding agency mock survey performance and communicates barriers and problems which are preventing successful achievement of survey excellence. • Drives quality patient care and compliance with the COPs and other applicable regulations through the completion of comprehensive mock surveys. • Drives survey readiness within each agency participating in the mock survey process. • Prioritizes patient and staff scheduling. • Meets scheduled timelines for completion of mock surveys, written statement of deficiencies and review of corrective action plans. • Communicates with the Administrator and leadership patient quality or safety concerns that could create immediate jeopardy. • Maintains confidentiality related to mock survey experience and findings. • Provides clear and accurate reports as requested. • Performs other duties/projects as assigned.
