Bluegrass Care Navigators - 100% Remote - no specific geographical location

Schedule - 8am-7pm EST with a rotation of 3 shifts one week, 4 shifts the next week. Total of 70 hours every two weeks. Every other weekend and rotational holidays.

As a BCN Hospice Team member, you will be a part of a team providing care and support to patients and their families who no longer receive treatments to cure their terminal illness, but instead choose care to improve their comfort and quality of life. While this often is a house or apartment, we also provide care at nursing homes, assisted living facilities, and hospitals. If a patient’s care needs are too complicated for home care or require a higher level of attention from nurses and other experts, we can provide care at one of our Hospice Care Centers.

About the role:

The Support Center RN is a member of the interdisciplinary team who work together to meet the physical, medical, psychosocial, emotional, and spiritual needs of BCN’s patients and families facing serious illness. The Support Center RN performs telehealth triage functions including but not limited to receiving calls; providing phone support; providing interventions utilizing BCN policies, procedures, and protocols; dispatching on-call nurses as needed; and making appropriate notifications to primary team regarding any changes in LOC. The Support Center RN completes documentation utilizing the EMR, point of service.

Requirements:

• Complies with accepted professional standards and principles.
• Able to cope with heightened family emotional stress, and tolerant of individual lifestyles.
• Complies with strict adherence to organizational policies, procedures, and protocols.
• Is currently licensed as an RN through the State Board of Nursing.
• Has excellent assessment, nursing judgment, and communication skills.
• Basic computer skills required. Advanced computer skills preferred.
• Is flexible and cooperative in fulfilling role obligation and has the ability to work with an interdisciplinary team.
• Is a graduate of an accredited school of professional (registered) nursing.
• Is self-directed with the ability to work with little supervision.
• Has at least five (5) years of nursing experience. Hospice, home health, Palliative, Primary Care or critical care/ED experience preferred. Telehealth experience also preferred.
• Employees will be expected to comply with Bluegrass Care Navigators Employee Health, infectious disease, and vaccination policies.
