Job Description
Transitions TSS
Transitions is a short-term, residential Transitional Support Services (TSS) program serving
individuals of all genders living with alcohol and/or substance use disorders and is funded and
licensed by the Bureau of Substance Addiction Services (BSAS), Department of Public Health
(DPH). The program is located in Boston, Massachusetts.

The model is designed to support individuals between detoxification and long-term residential
or outpatient care. Transitions utilizes the 'No Wrong Door Policy' and accepts clients from
detoxification, Clinical Stabilization Services (CSS), Recovery Homes, hospitals, homeless
shelters, outpatient settings, etc. The Boston Public Health Commission (BPHC) and Transitions are committed to protecting, preserving, and promoting the health and well-being of all Boston residents, particularly the most vulnerable.

Clients work with case managers to design and achieve goals on their individualized treatment plans that support long term recovery and overall wellness. Clients may explore external supports such as Medically Assisted Treatment (MAT), Community Support Providers (CSP), Recovery Coaches, therapy, etc. while developing aftercare plans to continue treatment. Daily, onsite group services are designed to help clients build skills, increase knowledge, reduce isolation, and develop healthier relationships to support recovery and prepare them for the next modality of care.

DUTIES
  • Ensures the overall safety of clients on the program premises by completing high quality rounds on the unit; engages in trauma informed milieu management.
  • Participate in required and assigned training.
  • Attend supervision and staff meetings.
  • Manages and distributes linen to clients.
  • Anticipates potential conflict situations and intervenes to deescalate conflict before, during and after crises.
  • Maintains accurate records utilizing an electronic medical record (EMR) including shift summaries, incident reports, etc.
  • Leads the hiring, onboarding and management for Fill-in Counselor relief pool.
  • Performs other duties as assigned.
  • Serves meals, ensures clean up after meals and kitchenette areas.
  • Communicates guidelines and expectations to clients in a trauma informed manner.
  • Provides clients with feedback and interventions in response to program guidelines.
  • Provides individual and/or group supervision to direct reports that promotes quality assurance/improvement and employee wellness.
  • Prepares and approves weekly payroll, ensuring time entries are accurate and appropriately billed.
  • Facilitate and monitor group meetings, Twelve Step meetings, onsite/offsite activities for clients.
  • Works collaboratively with program management to coordinate adequate staffing coverage and responds in a timely manner to employee coverage requests.
  • Assists the Unit Manager in managing inventory, ordering and distribution of food and other supplies.
  • Monitors and documents client medication self-administration.
  • Provides transportation in program vehicles of clients and supplies.

Minimum Qualifications

MINIMUM REQUIREMENTS
  • Demonstrated experience or related requirements as established by the program may be needed.
  • Advanced experience with knowledge of local community agencies and health care providers.
  • Excellent interpersonal skills and ability to communicate effectively.
  • Ability to understand, explain, apply the laws, rules, regulations, policies and procedures of unit activities - Ability to establish rapport with others and to maintain harmonious working relationships with others.
  • Demonstrated experience coordinating and implementing complex projects in the area of position.
  • Exceptional organizational skills with the ability of developing timelines and meeting deadlines.
  • Ability to write clear, concise reports, and to follow and to give written and oral instructions in a precise, understandable manner.
  • Bachelor's Degree with a minimum one (1) year supervisory experience and two (2) years office management experience OR College level course work equivalent to an Associate's degree with two (2) years supervisory experience and three (3) years office management experience OR high school diploma/GED and three (3) years supervisory experience and three (3) years office management experience.
  • Advanced knowledge & experience with MSOffice, MSWord/Excel, SAS with the ability to analyze and interpret data and willingness to learn other applications as needed.
  • Demonstrated cultural humility with diverse ethnic, cultural and socio-economic groups.
