Job Description

The Opportunity:
Saint Luke’s North Hospital – Barry Road is seeking a Social Worker to join our team! You will be responsible and accountable for the assessment of psychological, social, emotional, cultural, and financial needs, and the resulting treatment that impacts patients' health and recovery.
Shift Details: On-Call/Weekend/Holiday Rotation
The Work
  • Collaborates with multidisciplinary team to develop transition plan and address needs in a timely manner.
  • Provides professional social work services that utilize clinical knowledge to address the needs of patients and families during illness, crisis intervention, and post-acute discharge planning.
  • Participates in program planning and development to meet the needs of customers within the entity, health system and community.
