Job Description
This Social Worker position is 100% remote and candidates can apply from any state but must be able to work EST hours.

Normal Working Hours: Monday through Friday 8:00am-4:30pm EST.

No evenings, weekends, holidays, or travel will be required.

The Social Worker partners with a registered nurse case manager to utilize a collaborative process of assessment, planning, and advocacy to meet a member’s benefit plan and/or health needs. Responsibilities include researching available options and resources for members with financial, transportation, safety, and housing needs.

The Social Worker will work with a population spans the entire lifecycle.

All member contact is done telephonically or via electronic exchange.

The Social Worker's role is to empower members to make positive lifestyle and medical choices and works as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.

Pay Range
The typical pay range for this role is:
Minimum: 45,240
Maximum: 98,280

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Required Qualifications

  • Must have an unrestricted active license in state of residence
  • Candidate is required to have licensure: Licensed Masters Level Social Worker (LMSW)
  • Must be willing and able to obtain multi-state licensure (additional licensures will be paid for by the company).
  • 2+ years’ experience in medical social work

Preferred Qualifications
  • Telephonic Social Worker experience
  • Managed care/utilization review experience
  • Highly organized to manage continuously changing priorities
  • Ability to exercise independent and sound judgment in decision making
  • Proficiency with computers and an ability to navigate multiple systems simultaneously
  • Case management and discharge planning experience
  • Crisis intervention skills
  • Advanced care planning experience
  • Hospice/palliative care experience
