02200 Social Worker STRHS-Lawrenceburg 3.31.2017

Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.

Reports to: Director of Case Management

FLSA: Non-Exempt

Grade: 64

Essential Functions:
Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
Evaluates
Participates in case reviews to evaluate case management and progress for existing, new, and prospective patients. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
Discharge Planning
Creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. Completes psychosocial assessment and assists team with the development of the plan of care.
Supervises and/or trains new staff, students, and interns.
Performs Medicaid screenings as indicated for NHP.
Patient - Centered Care
The Case Management Social Worker is responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated.
Liaison
Acts as liaison between patient and community resources/post-acute care providers.
Assists Case Manager RN
Assists Case Manager RN in facilitating patient movement across the continuum of care and identifies, addresses, and track barriers to patient.
Patient Resource
Address patients' concerns and goals while maintaining constant communication. Provides education and or consultation to clients and families, treatment team and community.
Rapport
Build rapport with patients and their families.
Collaborates and Represents
Actively collaborates with team to meet patient care management and system improvement goals. Represents the Social Work elements on institutional and community committees/tasks forces. Provides leadership for department committees/workgroups.
Contributes
Contributes to the plan of care with knowledge of population specific needs as it relates to the identification of patient psychosocial needs. Maintains professional documentation which meets department and STRHS standards and compliance.
Process Improvement
Contributes to the organization by actively participating in process improvement initiatives and shared decision making.
General
The responsibilities listed are a general overview of the position and additional duties may be assigned.
Non-Essential Functions:
  • Maintains a neat and professional appearance in compliance to the existing dress code.
  • Accepts all call schedule as directed.
  • Attends and participates in staff meetings, in-service, projects and committees as assigned.
  • Perform other duties as assigned..
  • Adheres to and supports policies and procedures of the STRHS.
  • Works scheduled shifts including overtime, when necessary.

Job Requirements:
Minimum Education
X Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.
