Job Details
Scheduled: 8:30am - 4:30pm
$6,000 sign on bonus for eligible candidates

Provides palliative care patients and their families with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services address their social, emotional, and financial needs. Services provided include clinical assessment, crisis intervention, brief therapy, case consultation with the healthcare team and collaboration with other professionals in the community. These services advise family caregivers, provide patient education and counseling, and make necessary referrals to community resources for continuity of care. Assists with completion of IPOST and advance care planning documents and engages the palliative care patient and family in defining goals of care.
Qualifications
Bachelor’s degree in Social Work from a program accredited by the Council of Social Work Education or LBSW preferred
Bachelor’s degree in a Human Services-related field, ie. Psychology, Sociology, Criminal Justice, etc., required
Social Work Experience In Health Care Settings Preferred
Social Work experience in other practice settings preferred
Maintains valid licensure/certification/registration
