Job Description

Collective Health is looking for a full time, experienced, and forward - looking for a Social Worker to guide and support members facing complex conditions driven by data, crafted by evidence-based standard methodologies, and supported by premier customer service. The SW Care Managers are part of an interdisciplinary care team located in Collective Health's Plano, TX office. The SW Care Manager will ensure that our members' care is reliably coordinated.

We are looking for an experienced social worker who is adept at navigating people-to-care resources. The Senior Specialist, Social Worker would be responsible for providing telephonic care management, including, but not limited to:

  • Working collaboratively on co-managed cases with nurses, pharmacists, and dietitians
  • Participating in clinical interdisciplinary rounds
  • Leading outreach efforts to high-risk individuals with complex medical conditions through telephonic or electronic outreach
  • Providing emotional support, assisting with benefits and behavioral health navigation
  • Working cross functionally with other teams to assist with benefits and claims questions
  • Participating in program case rounds and providing input for process improvement opportunities and resources for team
  • Educating and empowering members using decision support through multiple modalities including telephone and email to help members achieve their health goals
  • Utilizing motivational interviewing to assist members to become self sufficient in their care planning
  • Will be required to take call as scheduled
  • Completing telephonic social work assessments and care plans for patients with behavioral, social, financial and environment problems
  • Coordinating a range of services, including community resources and health benefit resources
  • Required Travel: 2 days per quarter with client-facing and member-facing presentation and interaction
  • Identifying and discussing current barriers related to member's current condition, and provide interventions to address these barriers

You would be a good fit for the Senior Specialist, Social Worker position at Collective Health if:

  • You are excited to work in a telephonic social work role
  • Bilingual Preferred (English/Spanish)
  • Additional Case Management Certification is a plus
  • You have an MSW or LCSW from an accredited school of Social Work
  • A strong moral compass and commitment to Collective Health's core values of integrity, transparency and trust
  • You are tech savvy; able to talk, type, and navigate an electronic chart with ease
  • You are a problem solver using best practices to achieve goals
  • Experience in case management; experience at a payer is a plus
  • You are driven, able to carry a caseload and prioritize to complete daily tasks and outreach
  • You are okay with a role that does not involve providing individual therapy
  • Proficient in Google suite, such as Google Sheets, Docs, etc.
  • You do not require hours of supervision for licensure. Collective Health will not be able to provide supervision for clinical hours. Collective Health will not provide supervision or experience with Clinical Psychosocial Diagnosis, Assessment, Treatment, Individual or Group Psychotherapy.
  • You are passionate, empathetic, and excited to help people
  • You are independent, tenacious, and capable of identifying and implementing best practices
  • Discharge planning, complex care coordination, behavioral health, and/or home health, oncology, behavioral health, autism, pediatrics, and/or maternity experience a plus
  • You are comfortable with ambiguity and open to collaborating with people across different teams
  • You are capable of thinking creatively to implement care navigation solutions that challenge status quo

Pay Transparency Statement

This is a hybrid position based out of our Plano, TX office, with the expectation of being in office at least three weekdays per week. #LI-hybrid

The actual pay rate offered within the range will depend on factors including geographic location, qualifications, experience, and internal equity.
