Under minimum supervision the Community Care Navigator works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management.

Coordinates the care for complex designated patients upon discharge. Works collaboratively in a multidisciplinary team providing community based comprehensive case management services for special populations. The Navigator assists patients and families with psychosocial and environmental problems that may predispose toward illness or interferes with obtaining maximum benefits from medical care or reaching optimum level of physical functioning. Works as part of a professional care coordination team to provide skilled psychosocial services. Efficiently communicates with physicians, clients, caregivers and partner agencies in person, in writing and through use of diverse HIPPA compliant electronic communication platforms. Takes initiative and maintains the ability to make independent judgments.

Required qualifications:

• 2 years Social services experience in a community or health care setting.
• Bachelor's Degree in Social Work from an accredited School of Social Work, Human Services, Psychology or a related field from an accredited university.

Preferred qualifications:

• Some community based fieldwork experience providing assessments and interventions.
• Social service case management experience working with families and older adults who are coping with unstable housing, food security or other social determinants of health.
• Upon request: Required upon request: Driving is a necessary part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
• Master's Degree in Social Work from an accredited School of Social Work, Human Services, Psychology or a related field from an accredited university.
