Description

The RN Field Clinician position will provide nursing care which includes:

• Monitoring of patient's condition
• Health promotion and prevention
• Patient education
• Coordination of services
• Promotion of patient/family independence
• Comprehensive health and psychosocial evaluation

Responsibilities

• Prepare clinical notes and OASIS assessments
• Develop, initiate, implement, and update the plan of care as needed or at least every 60 days
• Evaluate the effectiveness and outcomes of care
• Observe and report patient's response to treatment and any changes in his/her condition to the physician and supervising registered nurse
• Initiate preventative and rehabilitative nursing procedures as appropriate for he patient's care and safety
• Coordinate services and inform physician and other personnel of changes in the patient's condition and needs
• Provide or supervise skilled nursing services in accordance with physician's orders
• Plan for the patient's discharge from home care services
• Assess and regularly re-evaluate the nursing needs of the patient
• Function as the patient advocate in all medical decisions affecting the patient

Requirements

• ER or ICU experience is preferred but not required
• Current CPR certification
• Must hold a current license issued by the Louisiana State Board of Registered Nurse Examiner's without restrictions
• Home Health experience is preferred but not required
• Must have a minimum of one year experience as a registered nurse
