This role focuses on improving the health status of and caring for individuals with chronic conditions through promoting preventative care and wellness, education on managing chronic disease processes through self-care, patient-centered care planning, discharge planning, health coaching, care coordination, and transitional care management processes. This position is Monday-Friday from 8:00 AM-4:30 PM, with no weekends, or on-call. This position is hybrid; three days are spent in the physician's office and two are at home.

Duties Include:

• Patient Education (telephonically or in the office)
• Annual Wellness Visits
• Chronic Care Management
• Preventative Care and Wellness Promotion
• 15 patient contacts per day via phone, office, etc.
• Post hospitalization Follow-Up ( Hospital Admissions and ER Visits)
• Vaccine Clinics
