This vacancy is located at SCLHSA's Administration Office located in Houma, LA. This position serves as the Performance Improvement, Integrated Care and Infection Control/Employee Health Coordinator for SCLHSA. Integrated Care is a healthcare delivery approach focusing on integrating primary care, behavioral health, other healthcare, and community and social support services. The position oversees and develops this comprehensive model while working collaboratively with the established primary care teams, psychiatrists, other SCLHSA clinical staff, outside referrals and community resources. The Infection Control Program protects employees in the workplace by reducing risks to exposures, and offering vaccinations, performing TB test at hire, and monitoring incidents. This position will ensure that SCLHSA staff members are educated about blood borne pathogens and TB, collect data and monitor all incidents related to infection control and/or infectious diseases. Receives direction from the Program Manager 1A.

30% Integrated Care/Health Home:
- Collaborates with Supervisor and the Executive Director in developing quality measures for disease management initiatives, such as hypertension, asthma, diabetes, and obesity.
- Coordinates and organizes education/training program for the Integrated Care program.
- Serves as coordinator of Integrated Care/Health Home.
- Identifies performance indicators to measure medical, behavioral, and functional outcomes of individuals.
- Develops and monitors policies, procedures, and activities that support access to services, care coordination, health promotion, and referral linkages for patients in the program.
- Ensures that comprehensive care management is provided to individuals receiving integrated care; including assessment of service needs, gaps in treatment, health status monitoring, and medication reconciliation. Works collaboratively with the Primary Care Nurse Practitioners to ensure all of items listed under comprehensive care management are received for the patients.
- Responsible for reviewing and submitting third party payor audits for review upon receipt of requests for medical records.
- Serves as the Patient Portal Coordinator for SCLHSA’s electronic health record system. Functioning as a resource for the staff and patients regarding patient portal/Healow set up and trouble shooting.
- Collects, aggregates, and analyzes individual healthcare data across SCLHSA populations and disease processes and uses the data to manage and improve outcomes for patients in the Integrated Care/Health Home.
- Analyzes data and prepares reports for the Deputy Director and/or Executive Director routinely and as needed.
- Responsible for pulling and reporting monthly PEC statistics for the Nursing Supervisors, Clinical Director, Deputy Director, and Executive Director.
- Performs monthly chart reviews to assure that essential, required documentation is present in the electronic health record to meet standards of medical necessity, payers, and those of agency and regulatory bodies;
- Assists the Deputy Director with nursing staff oversight, such as, conducting Nursing Supervisor meetings, participating in on-site monitoring and problem-solving
- Assists Supervisor with maintaining an integrated reviewing process that allows for maximum utilization of collected data in improving patient outcomes and resource utilization and for patients’ agency wide
- Serves as the agency HIV Coordinator which includes, reviewing policy and procedures, ensuring ongoing training is proved by the Office of Public Health, conducting chart audits and reporting on findings. Ensures that quality assurance is maintained at each Behavioral Health Center by performing annual site audits. Ensures that supplies need for HIV testing are maintained.
- Assists the Executive Director with implementation, coordination, and monitoring of the SCLHSA Performance Improvement program.
- Serves as resource consultant to other agency personnel regarding standards of care, SCLHSA policies/procedures and regulatory requirements
- Ensures compliance with the performance improvement initiatives required by CARF and LDH, as well as other agency initiatives
- Assists in the CARF Accreditation processes and surveys, as needed.
- Conducts quarterly reviews of HIV, HCV and TB administration in the Behavioral Health Centers and reports findings to Nurse Supervisors, Primary Care Practitioners, Clinical Director, and Deputy Director.
- Provides nursing duties in the Behavioral Health Centers as needed.
- Develops/conducts TB education to all SCLHSA staff annually.
- Plans, implements, and ensures annual Influenza vaccinations are provided at all Behavioral Health Centers.
- Plans and ensures that TB testing for personnel is completed upon hire. This includes administering test, reading results, documenting in appropriate system. If any staff member tests positive, educates employee and refers to health care provider.
- Reviews all incident reports related to Infection Control.
- Provides orientation and education related to Infection Control to new employees.
- Conducts special projects or intense studies of other potential organizational development activities at the request of the Deputy Director, Executive Director, the SCLHSA Board or LDH.
