Rn Case Manager - Home Care (Full-Time) Days

SUMMARY: The RN Case Manager is responsible for the initial assessment of the client / family for acceptance for home health services within 48 hours of admission, in accordance with state regulations. Must be a RN license. BSN preferred. RN will w ork at the client's home, skilled facility, or other place of residence providing services in accordance with agency policy. RN p erforms health care services for the client that reflects the agency's philosophy and standards of care, provides comprehensive client care following the physician and team's plan of care, and performs technical skills safely and in accordance with the agency's policies and procedures. Performs nursing care as defined within the Nurse Practice Act and stated agency policies and procedures. Reports To: Supervisor of Clinical Services Titles Supervised Licensed Practical Nurses, Certified Nursing Assistants Schedule: Days Full-Time 40 hrs. Flex. DUTIES Essential Responsibilities:

• Participates in the on call rotation. 25pletes admission and discharge of clients in a timely fashion.
• Act as the coordinator of the health care team in order to maintain continuum of care with all disciplines, including contracted services.
• Uses effective written / verbal communication.
• Charts progress notes, reviews client status and care provided. Provide progress notes to the patient's health care provider about the patients under care when the patient's conditions change to there is a deviation from the plan of care or at least every sixty days. 11plies with collection of required documentation for timely submission to the appropriate payor.
• Performs technical skills safely and according to accepted procedures.

• All other duties as assigned. QUALIFICATIONS Minimum Qualifications: Education Graduate of an accredited school of nursing program. BSN a plus. Must be registered with the state Board of Nursing. Must meet all requirements for the applicable state regulatory agency. Must have previous work experience as a Registered Nurse in a clinical environment. Ability to read, write and communicate in English. Experience 1+ years clinical experience in a hospital (ie. Cardiac, Emergency Room) or similar health care setting preferred. Licensure RN, State of Connecticut, CPR certification, a BSN preferred. We are currently seeking qualified candidates for a full time Registered Nurse (RN) Case Manager position in the Las Vegas area. : This is an out-patient position, that requires an independent and experienced RN. The RN-Case Manager is an integral part of the interdisciplinary care team and as such is responsible for admitting and coordinating appropriate care for each patient and family. RN-Case Manager main job duties include and are not limited to; admissions, assessments, discharges and pronouncements. • Demonstrates a thorough knowledge of and practices within the Infection Control Policies and Procedures established.
• Explains all procedures, treatments, medications, and client's plan of care to client and family, as appropriate.
• Directs the activities of the clinical care to LPNs, CNAs and other ancillary staff. Prepares written instructions and supervises the staff in the home, in accordance with policy, and state and federal regulations.

• Coordination of services with the patient, family, and others involved in the care plan.
• Responsible for the observation, assessment nursing diagnosis, counsel, care and health teaching of the sick, injured and infirmed, and the maintenance of the health and prevention of the illness of others. 6pletes initial nursing assessment within 48 hours of the client's admission and prepares care plan.
• Assists with restorative services including ambulation and related therapies. 23municates regularly with the Supervisor of Clinical Services.
• Demonstrates a working knowledge of equipment, wound care, vitals, injections, and case management. (preferred)
• Regular evaluation of patient's progress, prompt action when any change in the patient's condition is noted or reported, and termination of care when goals of management are attained.
• Administrers medications and treatments as prescribed by the client's physician.
• Documents and communicates client responses and the effectiveness of nursing / medical interventions in the client's clinical record.
• Demonstrates respect for client confidentiality and clients rights per state and federal regulations.
• Implementation or delegation of responsibility for twenty-four hours nursing service and home health aide services.
