Key Accountabilities

Resident Care
  • Aids residents in preparation for medical tests and doctor’s appointments as necessary. This includes proper completion of physician visit forms.
  • Assists residents with preparation and transport to/from appointments, activities, meals, and social programs as necessary.
  • Assists residents with Activities of Daily Living (ADL’s); i.e. dental and mouth care, dressing/ undressing, hair care, toileting, personal hygiene, and other duties as directed.
  • Checks residents routinely to ensure that their personal care needs are being met.
  • Answers resident emergency calls promptly.
  • Supports residents in walking with or without self-help devices as instructed.
  • Lifts, turns, moves, positions, and transports residents into and out of beds, chairs, showers, etc. as necessary.

Following Delegated Tasks

  • Observes, documents, and reports all changes in residents’ condition to the RN as soon as practical.
  • Watches for and reports any changes in suite temperature, ventilation, lighting, etc.
  • Reports all accidents and incidents and completes required documentation.

Customer Service

  • Informs oncoming shift with any new resident conditions.
  • Maintains a professional demeanor in demanding situations.
  • Works collaboratively with team members and all staff to solve challenges encountered with resident and team needs.

Requirements

  • Must possess the ability to make independent decisions when circumstances warrant action.
  • Stays up to date on all required training.
  • Must be a minimum of sixteen (16) years of age.
  • Follows all policies and procedures.
  • Must receive a "not disqualified" criminal background result and may not be on the OIG exclusions list.
  • Understands and adheres to the Vulnerable Adult and Abuse Policy. Is familiar with the Resident’s Bill of Rights and adheres to it.
  • Complies with state and federal HIPAA requirements.
  • Must be able to relate information concerning residents’ condition.
