Registered Nurse, Part Time Days - Hospice

Responsibilities:
The Hospice Primary Care Nurse assesses the needs of patients/resident and/or their significant others & develops a plan of care to meet these needs. The plan may be carried out in the home, residential health care facility or hospital. The major focus of the plan of care will be symptom management & pain control.
Qualifications:
Current New York State license to practice as a Registered Professional Nurse.
Current CPR/BLS
Graduate of an accredited Nursing Education program (2,3 or 4 year.)
Possesses physical assessment skills.
Recent Community Health/Hospice or Medical-Surgical Nursing experience
Ability to work with other groups to coordinate care.
Ability to provide own transportation must possess a New York State Driver’s License.
