The CPP functions at the highest level of clinical practice, working independently under their individual scope of practice to directly care for patients, as a member of their assigned team(s) and serves as a mid-level provider to initiate, modify, or discontinue medication therapy and as a consultant for intensive medication therapy management services. Duties include but not limited to: Clinical - Direct Patient Care Activities that require a scope of practice:

  • In collaboration with patient health care team, assess appropriateness and effectiveness of patient medication regimen by reviewing medical records, laboratory and other diagnostic study results, interdisciplinary communications, and interviewing/examining patient as appropriate.
  • Serves as an advanced practice provider within individual scope of practice. The CPP will review, initiate, modify, and monitor pharmaceutical care plans and orders within scope of practice.
  • Appropriately documents in the patient's medical record, including medication history, medical history, physical examination and ongoing patient specific monitoring of therapy, assessments and plans
  • Monitors and assesses therapeutic and adverse effects of drug therapy (including drug interactions) through selection and evaluation of physical and laboratory parameters and makes referrals as acute management problems arise

Education and Research:

  • Participates in the training of pharmacy residents and students.
  • Maintains professional competency by keeping up with information on new drugs, disease state management, and innovations in pharmacy practice and must demonstrate the knowledge and skills necessary to provide care appropriate for the area of practice specialty.
  • Participate in professional practice review as required to maintain scope of practice.
  • Provides formal and informal education, training, and drug information. Conducts in-service education programs for physicians, nursing, pharmacy staff, trainees, and others as assigned. Assists with orientation/education/training of staff new to facility.
  • Educates and trains other health care disciplines (e.g., housestaff, nurses, dietetics) on the safe, rational, and cost-effective use of medications.

Administrative and Organizational Support

  • Contribute to medical center committees as assigned to provide thoroughly investigated, unbiased, well-documented medication-related information.
  • Responsible for the operational status of the various aspects of pharmacy programs. CPP will be required to make administrative decisions in instances where immediate decisions are required. Pharmacy Policy and Procedures, VA Regulations, and State and Federal pharmacy laws will serve as operational guidance.
  • Participate in scholarly and QM activities to improve safe and effective medication use and clinical pharmacy functions.
  • Assist Pharmacy and Medicine Services in accomplishing the mission and goals as related to the provision of pharmaceutical care.
  • Consistently demonstrates a good awareness of safety procedures and regulations by quickly reporting or taking appropriate action to correct safety hazards anywhere in the Medical Center without regard to whether he/she has direct responsibility for the situation. Maintains a safe and orderly work area.
  • Contribute to the development and implementation of Pharmacy or medical center policies, protocols, or programs to improve medication usage and patient outcomes.
  • Intervention reporting: Documents workload, interventions, adverse reactions, safety events, and outcomes using appropriate methods as needed and on a periodic basis as required.

  • Completes all assigned tasks in a timely manner.
  • Interacts with the internal/external customers in a manner that proactively enhances customer service and satisfaction.

Basic Requirements:
  • Education
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)
(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
  • Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification.

May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). All licensed pharmacists employed in VHA in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure/certification/registration that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:
(1) Employees grandfathered into the GS-660 occupational series may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions.
(2) Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard. Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, that meet all the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation.
(3) If a licensed pharmacist who was retained under this provision leaves the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation.

Grade Determinations:
GS-13 Clinical Pharmacist Specialist
Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level.
Pharmacists assigned to this position must demonstrate the following KSAs to successfully perform the duties of the position, and that is typically in or related to the position to be filled:
a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Preferred Experience: Successful completion of PGY-1 pharmacy residency training, 3 to 5 years of clinical practice with evidence of provider level direct patient care within last 7 years and/or BPS Board Certification. Candidates should have recent experience in a current or previous job (including residency training) that includes direct patient care and comprehensive medication management responsibilities in an acute care setting. Preference will also be given to those who have experience delivering care to the target patient population.

The actual grade at which an applicant may be selected for this vacancy is GS-13.

Physical Requirements: See VA Directive and Handbook 5019. The position requires visual and auditory acuity at a level to provide safe and effective care. Use and function of all normal motor and sensory anatomy for provision of patient care, reading, writing, listening, observing/seeing, heavy lifting, bending, pulling, pushing, stooping, squatting, walking, standing, use of fine motor muscles. In an eight-hour day, must be able to sit for 3 hours, stand for 3 hours, and walk for 1 hour. The position also requires ability to work on computers for extended periods of time. Requires use of hands for actions such as typing, simple grasping, fine manipulation, and repetitive actions.
Requires the ability to frequently respond to multiple demands and priorities; emotional intelligence to respond to stressful and/or emergent situations and crises appropriately and calmly; self-care skills to maintain physical, psychological, and emotional health. Heavy lifting of 35 pounds and over, heavy carrying of 35 pounds and over, straight pulling, pushing, reaching above shoulder, use of fingers, both hands required, walking, standing, kneeling, repeated bending, ability for rapid mental and muscular coordination simultaneously. Has potential for exposure to infectious diseases, hazardous waste and chemicals.
