Job Description
Howard Community Health Center in Howard, SD is seeking a full-time Patient Support Specialist. The Patient Support Specialist educates and connects Horizon Health Care patients with identified social determinates of health to available resources and assist with navigating barriers. Successful candidates for the Patient Support Specialist should be passionate about providing high-quality, patient centered care.
