The Patient Access Team at Planned Parenthood of Greater Washington and North Idaho (PPGWNI) is on the lookout for a Patient Navigator II. This is a full-time position (37.5 hours/week) working Monday-Friday. This position can be partially remote, but we require candidates to live in the Central/Eastern Washington region and within a short commute to PPGWNI Health Centers.
The Patient Navigator II will support PPGWNI’s goal in improving access to treatment and care for vulnerable populations to improve their health status and outcomes. The Patient Navigator II performs a variety of health education, outreach, follow up, clerical, and social support services to our patients and outreach in the community. This position guides patients through referrals to other healthcare providers and related social and financial services to enable them to access services that improve their health status and quality of life.
Requirements:Core Responsibilities
- Maintain linkages with local community resources and organizations that can provide care or services not provided at PPGWNI to our patients
- Understand and maximize patient assistance programs and funding sources and assist patients in enrolling in these programs
- Identify barriers to patients accessing services and seek to address them
- Act as an advocate on behalf of the patient to ensure access to related services
- Maintain communication between patients, providers, follow up team, and community organizations
- Attend and participate in GWNI follow up team meetings, PPFA patient navigation and any other meetings relevant to patient navigation work
- Conduct follow up with patients to ensure they return for scheduled appointments
- Track and document all referrals in logs and in the EHR system
- Provide timely follow-up of client referrals in accordance with established guidelines.
- Develop a network with other patient navigators, referral coordinators and case managers in the healthcare and social service field
- Provide a main point of contact with outside community agencies, pharmacies, and vendors
- Report out on activities of interest to leadership team, Board, and funders
- Maintain Follow-Up Logs and audits to assist in ensuring appropriate patient care and to track productivity.
- Conduct outreach to communities that could benefit from our services
- Complete medication prior authorizations and coordinating with billing staff. Provide medication adherence support to patients.
- Provide patient notification and follow-up care as established by policies and protocols in a non-judgmental and non-biased manner
- Provide patients with information on healthcare resources and agencies and support their ability to access these services by making appointments and sending referrals
Required Qualifications
- Minimum of three years’ experience working with vulnerable populations as a case manager,
- Experience advocating for or navigating with individuals seeking any type of medical, behavioral health, or social support services.
- Ability to take initiative, utilize resources effectively and possess a problem-solving attitude
- Must have the ability to de-escalate high-anxiety situations and remain calm in the face of adversity.
- Must be well organized with the ability to prioritize with minimal supervision.
- Bachelor’s degree in social work, public health, sexuality, education or related field, or equivalent training and experience.
- Ability to use Microsoft Outlook and Office, including Excel and PowerPoint.
- Referral coordinator, or similar position in a healthcare or nonprofit setting.
- Must demonstrate excellent verbal and written communication skills and the ability to multi-task in a confidential and fast-paced environment of frequent change.
Preferred Qualifications
- Bilingual in English/Spanish.
- Experience with diverse race, ethnicity, nationality, sexual orientation, gender identity, socio-economic background of our patients.
Travel
- Travel will be to affiliate sites and other venues as needed. A valid driver’s license, vehicle insurance, and access to reliable transportation is required.
