Overview
The Manager, Patient & Provider Engagement will be responsible for the company's patient and provider engagement strategy and operations. Contessa creates strong relationships with patients and their providers. This position is pivotal in creating a positive experience for patients and physicians to consider care offered by the company. This position will work with Contessa's hospital partners, including their hospitals and physician practices. The leader will also work with clinicians and other home-based care team members to schedule visits in patients' homes. The leader will be accountable for both strategy and execution of patient and provider engagement, which will include managing staff performing engagement activities, including outbound calls and other ways to communicate with potentially eligible patients and their providers
Responsibilities
Work with markets and provider partners to develop strategies to increase referrals to the company • Increase inbound calls from patients and their caregivers
Hire and lead a team of engagement professionals that will speak with patients and providers
Create processes and scalable systems to obtain engagement results across multiple markets
Collaborate with the company's marketing leadership and partners to create patient and providerfacing content
Create a quality assurance program and management structure to oversee the quality of the department's work
Engage in process improvement activities using data from quality assurance plan
Report engagement outcomes to hospital partners, health plans and company management
Emphasize data-driven decision-making and culture to increase patient satisfaction and company performance
Establish and maintain formal and informal key performance indicators to track the impact and effectiveness of engagement team efforts
Provide coaching to team and develop team skills to achieve results
Collaborate with internal and external stakeholders to build onto the company's proprietary platform, Care Convergence
Qualifications
Bachelor's or equivalent experience in health care
Experience operating a scalable engagement team and developing and executing new and innovative provider and patient engagement strategies
Experience using a client engagement system
Exceptional customer/patient orientation
Excellent presentation skills
Proficient in the use of Microsoft Office suite of programs including Excel, PowerPoint, and Word
